In her report probing how Lucy Letby was able to commit her crimes, published Tuesday, Lady Justice Thirlwall found there had been a "complete failure" at all levels to invoke safeguarding procedures at the Countess of Chester Hospital, and concluded that some babies would have survived and some attacks prevented had action come earlier.
